Pharmacodynamics.
Azelaic acid, the active substance of Aknestop, is a naturally occurring straight-chain dicarboxylic acid, devoid of toxicity, teratogenicity and mutagenicity. It is believed that the therapeutic efficacy of Aknestop cream in the treatment of acne is determined by its antimicrobial effect and a direct effect on follicular hyperkeratosis. Clinically, there is a significant decrease in the density of colonization of Propionibacterium acnes and a significant decrease in the fraction of free fatty acids in the lipids of the skin surface. Azelaic acid in vitro and in vivo inhibits the proliferation of keratinocytes and normalizes the disruption of the processes of terminal differentiation of the epidermis during the formation of acne.
In a rabbit ear model, azelaic acid accelerates comedolysis of tetradecane-induced comedones. At a concentration of 0.31 – 2.5%, a biocidal effect is manifested after prolonged contact for 30-120 minutes against gram-positive and gram-negative aerobic bacteria, anaerobic bacteria and fungi. In vitro studies have not demonstrated the presence of azelaic acid resistant mutants in either Staphylococcus epidermidis or Propionibacterium acnes.
Pharmacokinetics.
After applying the cream, azelaic acid penetrates into all layers of human skin. Penetration occurs at a greater rate through the affected skin than through intact skin. After a single application to the skin surface of 1 g of azelaic acid (5 g of cream), 3.6% of the dose is adsorbed subcutaneously. Part of the acid adsorbed through the skin is excreted in the urine unchanged. The other part is degraded by β-oxidation into short chain dicarboxylic acids (C7, C5) have also been found in urine.
Indications
Treatment of acne (acne vulgaris) characterized by the presence of comedones, papules, pustules and small nodules.
Contraindications
Hypersensitivity to the active substance or to any of the excipients of the drug.
