Pharmacodynamics

Azelastine is a phthalazinone derivative of the structure, with a prolonged anti-allergic effect, selectively blocks H1-receptors. When applied to the mucous membrane of the eye, azelastine also has an anti-inflammatory effect.

Azelastine inhibits the synthesis or release of chemical mediators involved in the early and late stages of allergic reactions, such as leukotrienes, histamine, platelet-activating factor (PAF) inhibitors, and serotonin.

Evaluation of the ECG of patients who used large doses of azelastine for a long time shows the absence of a clinically significant effect of azelastine on the QT (QTc) interval.

There was no association between azelastine use and ventricular arrhythmias or ventricular flutter and fibrillation in patients receiving oral azelastine.
