Ambroxol enhances the secretion of pulmonary surfactant by direct action on type II pneumocytes in the alveoli and Clara cells in the bronchioles, and also stimulates ciliary activity, thereby facilitating the secretion of mucus and its excretion (mucociliary clearance). Activation of fluid secretion and increased mucociliary clearance facilitate mucus clearance and relieve coughing.

Pharmacodynamics

It has been preclinically proven that the active substance of the drug, ambroxol hydrochloride, increases the secretion of the glands of the respiratory tract. Ambroxol enhances the secretion of pulmonary surfactant by direct action on type II pneumocytes in the alveoli and Clara cells in the bronchioles, and also stimulates ciliary activity, thereby facilitating the secretion of mucus and its excretion (mucociliary clearance). Activation of fluid secretion and increased mucociliary clearance facilitate mucus clearance and relieve coughing.

There is information that ambroxol hydrochloride blocks neuronal sodium channels; binding was reversible and concentration dependent.

Ambroxol hydrochloride has shown anti-inflammatory effects in vitro. Thus, ambroxol hydrochloride significantly reduces the release of cytokines from mononuclear and polymorphonuclear blood cells and tissues.

From studies involving patients with pharyngitis, a significant reduction in pain and redness in the throat with the use of the drug has been proven.

The use of ambroxol hydrochloride increases the concentration of antibiotics (amoxicillin, cefuroxime, erythromycin and doxycycline) in bronchopulmonary secretions and sputum.

Indications

Secretolytic therapy in acute and chronic bronchopulmonary diseases associated with impaired bronchial secretion and weakening of mucus movement.

Contraindications

Ambroxol should not be used in patients with known hypersensitivity to ambroxol hydrochloride or to other components of the drug.

Directions

1 ml solution for oral administration and inhalation = 25 drops.

Solution for inhalation

Adults and children over 6 years old: 1-2 inhalations of 2-3 ml of solution per day.

Children under 6 years: 1-2 inhalations of 2 ml of solution per day.

The drug can be used in all modern inhalation devices (with the exception of steam inhalers).

It should be diluted 1:1 with saline to ensure optimal air humidification released by the machine.

Do not mix with cromoglycic acid. It should also not be mixed with other solutions in which the pH of the solution exceeds 6.3, such as alkaline inhalation salt (Emser Salt). Due to the increase in pH, an increase in the ability to precipitate the free base of ambroxol hydrochloride or clouding of the solution may occur. It is generally recommended that Ambroxol solution for inhalation to be warmed to body temperature prior to inhalation. Given that the inhalation process itself can provoke a cough, it is recommended that patients breathe normally during inhalation. Patients with bronchial asthma before inhalation should use bronchospasmolytics to open the lungs.

Solution for oral administration

Adults and children over 12 years old: 4 ml 3 times a day for the first 2-3 days, is 90 mg of ambroxol per day, then 2 ml 3 times a day, which is 45 mg of ambroxol per day. The dosage of 4 ml 3 times a day can be continued after consultation with the doctor:

  • children 6-12 years old: 2 ml 2-3 times a day, which is 30-45 mg of ambroxol per day
  • children aged 2 to 6 years: 1 ml (25 drops) 3 times a day, which is 22.5 mg of ambroxol per day
  • children under 2 years old, 1 ml (25 drops) 2 times a day, which is 15 mg of ambroxol per day.

The drug can be diluted in water, tea, fruit juice, milk. Can be taken with or without food.

Ambroxol solution for inhalation should not be used for longer than 4-5 days without consulting a doctor.

In acute illness, you should consult your doctor if the symptoms do not disappear or worsen despite taking the drug.
