ENAM COMPOSITION
active substance: enalapril
1 tablet contains enalapril maleate 2.5 mg or 5 mg or 10 mg
Excipients: lactose, maleic acid, zinc stearate (tablets 2.5 mg and 5 mg) lactose, zinc stearate (tablets 10 mg).

round tablets of white or almost white color, with a chamfer, marked with 2.5 or 5, or 10 and a notch on one side and EMT marking on the other.

PHARMACOLOGICAL PROPERTIES
Pharmacological.
Enalapril maleate is a salt of maleic acid of enalapril, a derivative of the two amino acids L-alanine and L-proline.
Mechanism of action. ACE – peptidyl dipeptidase, which catalyzes the conversion of angiotensin I into the pressor substance angiotensin II. After absorption, enalapril is hydrolyzed to enalaprilat, which inhibits ACE. The suppression of ACE leads to a decrease in the level of angiotensin II in the blood plasma, which leads to an increase in plasma renin activity (suppression of the negative feedback between the activity of angiotensin II and the release of renin) and a decrease in aldosterone secretion.
ACE is identical to kininase II. Thus, enalapril can also block the breakdown of bradykinin, a potent vasodepressor peptide. However, the significance of this phenomenon for the therapeutic effect of the drug remains unclear. The mechanism by which enalapril lowers blood pressure is primarily associated with inhibition of renin-angiotensin-activity. Enalapril may exhibit an antihypertensive effect even in patients with low-grade hypertension. The use of enalapril in the case of arterial hypertension leads to a decrease in blood pressure in patients in horizontal and vertical positions without a significant increase in heart rate.
INDICATIONS
Treatment of arterial hypertension.
Treatment of clinically significant heart failure.
CONTRAINDICATIONS
Hypersensitivity to enalapril, to any excipient or to any other ACE inhibitor.
A history of angioedema associated with the use of ACE inhibitors.
Hereditary or idiopathic angioedema.
INTERACTION WITH OTHER DRUGS AND OTHER INTERACTIONS
Antihypertensive therapy.
The concomitant use of these drugs can enhance the hypotensive effect of enalapril. Concomitant use with nitroglycerin, other nitrates, or other vasodilators may further lower blood pressure.
Potassium-sparing diuretics or potassium supplements.
Diuretics (thiazide or loop diuretics).
Antidiabetic drugs.
Tricyclic antidepressants / antipsychotics / anesthetics / hypnotics.
Non-steroidal anti-inflammatory drugs, including selective COX-2 inhibitors. Non-steroidal anti-inflammatory drugs (NSAIDs), including selective COX-2 inhibitors (COX-2 inhibitors), can reduce the effect of diuretics and other antihypertensive drugs. Therefore, the hypotensive effect of angiotensin II receptor antagonists or ACE inhibitors can be attenuated by NSAIDs, including selective COX-2 inhibitors.
