Lopirel is used to prevent the manifestations of atherotrombosis:
- in individuals who have suffered myocardial infarction, ischemic stroke in diagnosed peripheral artery damage (vascular atherotrombosis, lower extremity artery damage);
- in a method with acute coronary syndrome: without lifting S – T (non-Q infarction, unstable angina), including in persons who underwent bypass surgery during coronary angioplasty; with a rise of S – T (in combination with acetylsalicylic acid).

Description
Lopirel 75 composition
active ingredient: clopidogrel;
1 tablet contains clopidogrel of bisulfate 97.86 mg, equivalent to 75 mg of clopidogrel;
auxiliary substances: anhydrous lactose, microcrystalline cellulose, crospovedon, glycerol dibegenate, talc;
shell: Opadry II 85G34669 pink (polyvinyl alcohol, talc, macrogol 3350, lecithin, titanium dioxide (E 171), iron oxide red (E 172)).
Dosage form
Film coated tablets.
Pharmacological properties
It’s an antithrombotic. This drug disrupts platelet aggregation (combining) by blocking the process of binding adenosine diphosphate to receptors located on the platelet shell, as well as activating glycoprotein IIb/IIa receptors. The drug also helps reduce platelet aggregation caused by other agonists, inhibits their activation by freed adenosine diphosphate.
Indications
Lopirel 75 is used to prevent the manifestations of atherotrombosis:
- in individuals who have suffered myocardial infarction, ischemic stroke in diagnosed peripheral artery damage (vascular atherotrombosis, lower extremity artery damage);
- in a method with acute coronary syndrome: without lifting S – T (non-Q infarction, unstable angina), including in persons who underwent bypass surgery during coronary angioplasty; with a rise of S – T (in combination with acetylsalicylic acid).
Lopirel is also indicated for the prevention of thromboembolic, atherotrombotic events in atrial fibrillation.
Contraindications
You can’t use Lopirel 75:
- in liver failure;
- clopidogrel allergy;
- in case of intolerance of auxiliary components;
- acute bleeding;
- children.
With caution, Lopirel 75 is prescribed to patients with chronic renal dysfunction, a risk of bleeding, a hereditary decrease in the function of the isoenzyme CYP2C19, as well as patients receiving acetylsalicylic acid, heparin, nonsteroidal anti-inflammatory, glycoprotein IIb/IIa inhibitors.
Children
Clopidogrel should not be used in children, since there is no data on the effectiveness of the drug for this age category of patients.
Use in pregnancy and breastfeeding
During these periods, Lopirel is not appointed.
Method of use and doses
Lopirel 75 is used orally, regardless of food.
Standard dose – 75 mg once a day.
- With ACS without lifting the S-T segment, a single load dose of 300 mg is started, and then 75 mg once a day. When used with acetylsalicylic acid, the risk of bleeding increases, so it is not recommended to exceed the dose by 100 mg. The maximum effect was usually achieved after 3 months of treatment.
- In an ACS with S-T segment rise, 75 mg/day is prescribed, starting with a load single dose of 300 mg. Treatment of a person over 75 years of age begins without a loading dose.
- In atrial fibrillation, a disposable 75 mg is used.
Overdose
Signs of overdose: prolongation of bleeding time, development of hemorrhagic complications.
Treatment: stopping bleeding, transfusion of platelet mass.
Side effects:
- Changes in blood indices: eosinophilia, neutropenia, leukopenia, thrombocytopenic purple, granulocytopenia, pancytopenia, aplastic anemia, agranulocytosis, severe thrombocytopenia, anemia.
- Immune disorders: serum disease, cross-hypersensitivity with thienopyridines (prasugrel, ticlopidine), anaphylactoid reactions.
- Mental disorders: confusion, hallucinations.
- Neurological disorders: paresthesia, intracranial bleeding, headache, dizziness, change in taste.
- Ophthalmological disorders: ocular, conjunctival, retinal bleeding.
- Otolaryngological disorders: vertigo.
- Cardiovascular disorders: hematoma, significant hemorrhage, vasculitis, bleeding from an operating wound, arterial hypotension.
- Respiratory disorders: interstitial pneumonitis, nasal bleeding, pulmonary bleeding, hemorrhage, bronchospasm, eosinophilic pneumonia.
- Digestive disorders: gastrointestinal bleeding, abdominal pain, diarrhea, ulcerative lesions, gastritis, nausea, meteorism, constipation, retroperitoneal hemorrhage, retroperitoneal or gastrointestinal bleeding, pancreatitis, ulcerative or lymphocytic colitis, hepatitis, stomatitis, abnormal results of liver function
- Dermatological disorders: subcutaneous hemorrhage, itching, bullous dermatitis, rash, purple, angioedema, exfoliative rash, drug-induced hypersensitivity syndrome, urticaria, DRESS syndrome, flat lichen, eczema.
- Musculoskeletal disorders: arthralgia, arthritis, hemarthrosis, myalgia.
- Urinary disorders: glomerulonephritis, hematuria, increased blood creatinine levels.
- General: fever.
Terms and conditions of storage
Store Lopirel 75 at 15-25 degrees Celsius, the maximum storage duration is three years.
