Description

Mirena composition
The main active ingredient of the drug is levonorgestrel.
The auxiliary substances are: elastomer polydimethylsiloxane, polyethylene, barium sulfate, colloidal anhydrous silicon dioxide, iron oxide.
Each such system contains 52 mg of levonorgestrel, which is released directly into the uterine cavity at a rate of 20 mg per day.

Release form
Produced in the form of intrauterine systems.

Pharmacological properties
Mirena belongs to a group of intrauterine systems containing a hormonal drug.
The main active ingredient is levonorgestrel, which is a progestogen with antiestrogenic activity. It is often used in gynecology as a contraceptive.
The use of this drug in the form of an IUD (intrauterine system) leads to the fact that levonorgestrel is released directly in the uterus, which allows the use of extremely small doses of the substance, since it goes directly to the target organ.
The effectiveness of levonorgestrel is due to the fact that the drug has the property of interfering with the fertilization of the egg due to inhibition of ovulation. It is especially effective in the first phase of the cycle, before the maturation of the egg in the ovary.
It also causes changes in the endometrium of the uterine cavity, which prevents the implantation of a fertilized egg, makes the cervical mucus more viscous.
Studies carried out on 17,000 women have shown its high effectiveness. Only in 0.7% of cases, when used within 5 years of research, an unwanted pregnancy occurred.

Indications
Mirena is used as a hormonal contraceptive. Also used for:

  • idiopathic menorrhagia;
  • hypermenorrhea;
  • dysmenorrhea;
  • local therapy with progestogens with estrogen replacement therapy.

Contraindications
Not applicable if the patient has hypersensitivity (allergy) to one of the components that make up the drug.
Also, use is contraindicated for:

  • progestagen-dependent tumors, incl. breast cancer;
  • diseases of the pelvic organs;
  • cervicitis, cervical dysplasia;
  • genital tract infections;
  • postpartum endometritis;
  • infected miscarriage or abortion in the past 3 months;
  • malignant tumors of the cervix or uterus;
  • uterine bleeding;
  • fibroids, leiomyomas;
  • liver disease, including liver tumors.

Children
The use of this drug is not recommended until the age of 16.

Application during pregnancy and lactation
It is not used to treat pregnant women.

Method of administration and dosage
The Mirena system is inserted into the uterine cavity in a medical facility by an experienced physician, and produces a contraceptive effect for 5 years, after which it is removed.
The IUD can be inserted into the uterus within 7 days after the onset of menstruation. You can replace the Mirena system with a new system on any day of the menstrual cycle. The system can be entered immediately after an abortion.
Postpartum administration should be postponed until complete involution of the uterus, but not earlier than 6 weeks after delivery.
If involution is too slow, it is advisable to consider introducing the system 12 weeks after delivery. In case of difficulties with the introduction of the system and / or unusual pain or bleeding during or after insertion, a physical examination and ultrasound should be performed immediately in order to exclude uterine perforation.

Overdose
Overdose has not been observed at the moment.

Side effects
When using this drug, an adverse reaction may occur in the form of:

  • headache, increased fatigue;
  • nausea, vomiting, bloating, abdominal pain, constipation, dyspeptic symptoms, pancreatitis, jaundice;
  • swelling of the mammary glands, pain in the mammary glands, pain in the small pelvis, in the back;
  • alopecia, hirsutism, skin hyperpigmentation, edema;
  • perforation of the uterine wall;
  • increase in body weight.

Storage conditions and periods
The shelf life of the drug is no more than 3 years from the production date indicated on the package.
