Multak is taken as an antiarrhythmic agent.
Designed to maintain sinus rhythm of the heart muscle after successful cardioversion in clinically stable adult patients with paroxysmal or persistent atrial fibrillation.
Prescribe only after considering alternative treatment approaches.
Description
Multak composition
The main active ingredient of the drug is dronedarone.
Excipients are: hypromellose, corn starch, crospovidone, poloxamer, lactose monohydrate, colloidal anhydrous silicon dioxide, magnesium stearate.
Each tablet contains 400 mg of dronedarone hydrochloride.
Release form
Produced in tablet form.
Pharmacological properties
Multak belongs to the group of antiarrhythmic drugs.
Its main active ingredient, dronedarone, is a multichannel blocker.
It has the property of inhibiting potassium channels, causing an extension of the action potential of the heart muscle and refractory periods.
Able to inhibit sodium and calcium channels.
Non-competitive adrenergic receptor antagonist (class II). In animal tests, prevention of atrial fibrillation or restoration of normal sinus rhythm has been recorded.
The drug prevents ventricular tachycardia and ventricular fibrillation in several animal models. This action is most likely due to its electrophysiological properties inherent in all IV classes.
Indications
Multak is taken as an antiarrhythmic agent.
Designed to maintain sinus rhythm of the heart muscle after successful cardioversion in clinically stable adult patients with paroxysmal or persistent atrial fibrillation.
Prescribe only after considering alternative treatment approaches.
Contraindications
Not applicable if the patient has hypersensitivity (allergy) to one of the components that make up the drug.
Should not be prescribed to patients with left ventricular systolic dysfunction, or patients who suffer from heart failure or have episodes of heart failure in history.
Does not apply when:
- atrioventricular block II or III degrees, complete left bundle branch block, distal blockade, sinus node dysfunction, atrial conduction disorder or sick sinus syndrome (except for the use of a pacemaker);
- bradycardia;
- persistent atrial fibrillation;
- unstable hemodynamics;
- left ventricular heart failure;
- systolic dysfunction;
- severe hepatic and / or renal failure.
The drug should not be used concomitantly with cytochrome inhibitors (ketoconazole, itraconazole, voriconazole, posaconazole, telithromycin, clarithromycin, nefazodone and ritonavir).
Also, they are not used together with drugs that cause tachycardia, tricyclic antidepressants, terfenadine and some macrolides (in particular, erythromycin), etc.
Children
It is not used in pediatrics.
Application during pregnancy and lactation
When treating pregnant women, it is not recommended to exceed the recommended dosage.
During treatment, lactation (breastfeeding) should be suspended.
Method of administration and dosage
Multak is taken orally with water, 1 tablet of 400 mg twice a day – in the morning and in the evening, during meals. Use on an outpatient basis is allowed.
Pre-stop the use of class I and III antiarrhythmics (flecainide, propafenone, quinidine, disopyramide, dofetilide, sotalol, amiodarone).
Do not drink the drug with grapefruit juice.
If you skip a dose, the next dose is not doubled, it is taken as usual.
Overdose
Usually does not cause intoxication. In theory, iron overdose can increase side effects.
Dialysis is ineffective. No antidote has been identified.
Treatment of symptoms, gastric lavage, and sorbent intake are recommended.
Side effects
Usually, this drug is well tolerated, and the side effects that appear are usually mild.
With individual drug intolerance, hypersensitivity reactions are possible in the form of:
- congestive heart failure;
- increased serum creatinine levels;
- nausea, diarrhea, vomiting, dyspepsia;
- deterioration of the liver;
- rash, pruritus, eczema, erythema, dermatitis;
- increased fatigue, asthenia;
- vasculitis;
- anaphylactic shock, angioedema.
If you have any unusual side effects as a result of taking this drug, you should consult your doctor about possible changes in the treatment regimen.
