Nifecard-XL composition
Main active ingredient: nifedipine.
Auxiliary components: cellulose, starch, lactose, magnesium stearate, silicon dioxide, macrogol. talc, hypromellose, titanium dioxide, etc.
1 tablet contains 30 mg of nifedipine.

Release form
Produced in tablet form.

Pharmacological properties
Nifecard-XL belongs to the group of cardiopreparations, which are created on the basis of pharmaceutical substances of the group of antimetabolites, purine structural analogs.
Nifedipine, which is the main active ingredient of the drug, is a calcium antagonist that is able to inhibit the flow of calcium ions through the walls of cell membranes by using a slow calcium channel into cells.
Once in the myocardium, nifedipine promotes the expansion of the coronary arteries, this is especially important for large blood supplying blood vessels. Also, there is an expansion of intact segments of the walls of blood vessels, which have already undergone partial stenosis.
Under the influence of nifedipine, coronary blood flow increases, blood pressure decreases, and a possible angiospasm is prevented.
Course use of the drug prevents the appearance of plaque deposits of atherosclerosis on the walls of blood vessels, reduces the need for the heart muscle in the supply of oxygen.
It is an effective means of preventing or reducing peripheral vasospasm.

Indications
Nifecard-XL is used to treat hypertension.

Contraindications
The drug in question is contraindicated for use in cases where the patient has severe hypersensitivity (allergy) to the main or to one of the auxiliary components.
Use is contraindicated in patients diagnosed with diseases such as:

  • cardiogenic shock;
  • unstable angina;
  • acute myocardial infarction (first 4 weeks).

Should not be used concomitantly with rifampicin.

Application during pregnancy and lactation
This drug is contraindicated in the treatment of pregnant women in the first trimester of pregnancy.
If it is necessary to administer the drug during breastfeeding, then lactation should be suspended for the duration of treatment.

Method of administration and dosage
Usually Nifecard-XL is started orally after a 3-day infusion. The tablets are swallowed without chewing, washed down with a sufficient amount of water.
You should avoid taking grapefruit juice, as it slows down the absorption of the drug, reducing its effectiveness.
It is recommended to take 1 tablet once a day. If necessary, the dosage can be increased to 60 mg (taken once a day).
Cancellation of the drug is made by gradually reducing the dosage and lengthening the interval of administration.
Elderly patients and patients with impaired liver function should reduce the dosage of the drug to the minimum effective.
Patients with impaired renal function do not need dosage adjustment.

Overdose
In cases of overdose by 4 times, 36% of patients experienced severe central nervous system intoxication (including blindness), which led to coma and death.
Overdose symptoms:

  • bradycardia, hyperglycemia;
  • metabolic acidosis, hypoxia;
  • cardiogenic shock, pulmonary edema.

It is recommended to use beta-adrenergic agonists to stop bradycardia, and when the heart rate slows down, artificial drivers of the heart.
For hypotension, calcium supplements are recommended.
Hemodialysis is ineffective.

Side effects
The drug causes the following side effects:

  • allergic reactions (angioedema);
  • anxiety, sleep disturbance;
  • headache, vertigo, migraine, dizziness, tremor;
  • visual impairment;
  • arterial hypotension, tachycardia;
  • nosebleeds, nasal congestion;
  • pain in the stomach and intestines, nausea, dyspepsia, bloating, dry mouth;
  • muscle cramps, joint swelling;
  • polyuria, dysuria;
  • erectile disfunction;
  • thrombophlebitis, pain at the site of the infusion needle.

Storage conditions and periods
The shelf life is up to 3 years.
Storage temperature – no higher than 25 ° С.
