Pharmacological properties
Subject to the recommended dosage, the drug suppresses ovulation, as well as fertilization, but only on condition that sexual intercourse occurred during the ovulatory period, when the probability of conception is greatest. It can also cause changes in the cells in the lining of the uterus (endometrium) that prevent implantation (attachment of a fertilized egg). This medication is ineffective if implantation has already occurred. This drug can prevent pregnancy in about 85% of cases. The longer the time elapsed between intercourse and the first pill intake, the lower its effectiveness (within 24 hours – the effectiveness is 95%, from 24 to 48 hours – 85%, from 48 to 72 hours – 58%). Therefore, it is recommended to start taking the pills as early as possible (but no later than 72 hours) after the sexual intercourse (unprotected). At the recommended dosage, it does not affect the metabolism of carbohydrates, fats, blood clotting factors.
Postinor is prescribed for postcontact (emergency) contraception after unprotected sexual intercourse, as well as in case of unreliability of the contraceptive method used.

Indications
Postinor is an emergency contraceptive medicine. Its active component (levonorgestrel) is an artificial progestogen with a contraceptive effect, pronounced antiestrogenic properties.

Contraindications
You can not prescribe the drug if you are allergic to the components of the drug. It is not used in adolescents under 16 years of age, as well as in severe liver dysfunction, pregnancy.
The tablets contain lactose monohydrate, so they should not be taken by people with hereditary or acquired diseases associated with lactose intolerance (lactase deficiency, glucose-galactose malabsorption).
It is prescribed with caution for Crohn’s disease, diseases of the biliary tract, liver, jaundice (including in the past), lactation.

Application during pregnancy and lactation
Postinor should not be used by pregnant women. When pregnancy occurs against the background of the use of an emergency method of contraception (based on available data), no adverse effects of the drug on the fetus have been identified.
Because levonorgestrel is able to pass into human milk, breastfeeding should be discontinued for 24 hours after taking this medication.

Method of administration and dosage
The drug is intended for oral administration (by mouth).
Postinor can be used exclusively for emergency contraception. Repeated use within one menstrual cycle is not recommended.
You must take two tablets in the first 72 hours after intercourse. The second pill is taken 12 hours later (not later than 16 hours) after taking the first pill.
To achieve a reliable effect, both tablets must be taken as soon as possible after unprotected intercourse (not later than 72 hours).
If vomiting occurs within 3 hours after taking the pills, you need to take another pill.
Postinor can be used at any time of the menstrual cycle. In case of irregular menstruation, pregnancy must be excluded first.
After taking an emergency contraceptive before the next menstrual period, barrier contraceptive methods (cervical cap, condom) must be used. It is not recommended to use this medication for repeated unprotected intercourse during one menstrual cycle due to the possibility of an increase in the frequency of acyclic bleeding, bloody discharge.
With the simultaneous administration of drugs-inducers of liver enzymes, the metabolism of levonorgestrel is accelerated. The following pharmacological agents can also reduce the effectiveness of the drug: amprenavir, nevirapine, lansoprazole, oxcarbazepine, topiramate, tacrolimus, tretinoin, barbiturates (phenytoin, primidone, carbamazepine); preparations containing Hypericum perforatum (St. John’s wort), as well as rifabutin, rifampicin, tetracycline, griseofulvin, ampicillin, ritonavir.
This medication can reduce the effectiveness of hypoglycemic (hypoglycemic), anticoagulant (phenindione, coumarin derivatives) drugs. It also increases the plasma concentration of glucocorticosteroids. Pharmacological agents containing levonorgestrel may increase the risk of cyclosporine toxicity by inhibiting its metabolism.
Women taking these medicines should consult a doctor.
The effectiveness of the pills after intercourse, in which contraceptives have not been used, decreases over time: with the time interval between intercourse and taking pills equal to 24 hours or less, the effectiveness of the drug is 95%, 25-48 hours – 85%, 49-72 hours – 58%.
Postinor does not replace the use of permanent methods of contraception. In most cases, the medicine has no effect on the nature of the menstrual cycle, however, acyclic (between menstruation) bleeding is allowed, and menstruation is delayed by several days. In case of delay in menstruation for more than 5-7 days or a change in its nature (abundant / scarce discharge), it is necessary to exclude the presence of pregnancy. The appearance of cramps in the lower abdomen, impaired consciousness (fainting) may indicate an ectopic (ectopic) pregnancy.
For adolescents under 16 years of age, pills are prescribed only in exceptional cases (for example, in case of rape) and only after consulting a gynecologist (to confirm / exclude pregnancy).
After emergency contraception, it is recommended to consult a gynecologist to select the most appropriate permanent method of contraception.
It should be borne in mind that emergency contraception does not protect against sexually transmitted infections.
With functional disorders of the gastrointestinal tract (with colitis, Crohn’s disease), the effectiveness of this medication may be reduced.

Overdose
In case of overdose, an increase in the severity of side reactions was observed. There is no specific antidote for levonorgestrel. Overdose treatment is symptomatic.

Allergic reactions are possible: rash, urticaria (urticaria), itching, swelling of the face. There may also be transient adverse reactions that do not require drug treatment (vomiting, loose stools, breast tenderness, fatigue, dizziness, cramps in the lower abdomen, dizziness, spotting / bleeding outside of menses).
There may be a delay in menstruation for no more than 5-7 days, if menstruation is delayed for a longer period, pregnancy should be excluded.
