Composition

active substance: metformin hydrochloride;

1 film-coated tablet contains: metformin hydrochloride 850 mg;

Excipients: hypromellose, povidone (K 25), magnesium stearate, polyethylene glycol 6000, titanium dioxide (E 171).

Dosage form

Film-coated tablets.

Pharmacological properties

The action of metformin is due to three mechanisms:

1) reduction of glucose production in the liver due to inhibition of gluconeogenesis and glycogenolysis;

2) increase muscle sensitivity to insulin, improve glucose uptake by peripheral tissues and its utilization;

3) slowing down the absorption of glucose in the intestine.

Metformin stimulates intracellular glycogen synthesis by affecting glycogen synthetase. Metformin improves the functional activity of all currently known types of glucose transporters (GLUT).

Metformin belongs to a group of biguanides that have antihyperglycemic activity and help lower blood glucose levels both on an empty stomach and after meals. The drug does not stimulate insulin production, so it does not cause hypoglycemia.

Metformin has a beneficial effect on fat metabolism, namely – its use in therapeutic doses lowers total cholesterol, low-density lipoprotein and triglycerides.

Indications

Siofor is one of the most popular medicines in the world, which is used to treat type II diabetes mellitus. A hypoglycemic drug belongs to the group of antidiabetic drugs (drugs to combat diabetes mellitus) of type II, non-insulin dependent. It is also effective in diabetes complicated by obesity, especially if diet does not help.
Siofor’s active ingredient – metformin – not only does not affect the amount of insulin in the blood, but also changes its quality. It has the ability to influence the mechanism for stimulating glucose uptake in muscle and adipose tissue.
It also increases blood circulation in the liver by accelerating the conversion of glucose to glycogen. Has the ability to induce a mild appetite-reducing effect (anorexigenicity), helping patients to follow a diet and slowing down the absorption of carbohydrates.

Contraindications

Cannot be prescribed if the patient consumes less than 1,000 kilocalories per day.
It is forbidden to take with type I diabetes mellitus (insulin dependent), because when taken simultaneously with insulin, it can cause hypoglycemia, coma. It will also have no effect on completely stopping insulin production in type II diabetes. It is contraindicated to use if the patient has symptoms of diabetic ketoacidosis, diabetic precoma, coma, myocardial infarction, heart failure, severe infections, impaired liver and kidney function, lung diseases, operations and injuries, catabolic diseases, the presence of tumors of various etiologies, alcoholism, and, of course , if you are allergic to metmorphine.

Method of administration and dosage

Prescribing a dose of Siofor 500 depends on the blood glucose level of a particular patient. They usually start with 1 tablet a day, gradually increasing the dose once a week (by 1 tablet in a dosage of Siofor 500 or 0.5 tablets Siofor 1000). When increasing the dosage, it is permissible to use 1 tablet of Siofor 850, then go to the dosage of 1000. Do not increase the dose by more than 1 tablet per week, in order to avoid an undesirable intestinal reaction.
The maximum dose per day is 3 grams per day, this is 6 Siofor 500 tablets or, preferably, 3 Siofor 1000 tablets.
It is taken with meals with water.

Overdose

The most common consequence of an overdose of Siofor 500 is an attack of glycemia, which can result in a coma (a sharp drop in blood glucose to a critical level). Symptoms of hypoglycemia: tachycardia, trembling in the limbs, increased sweating, blurred consciousness. In this case, immediately give the patient a sugar solution, in case the patient has lost consciousness, inject a glucose solution intravenously. After regaining consciousness, feed the patient with carbohydrate food.
It is also possible to develop lactic acidosis, the symptoms of which are nausea, vomiting, weakness, rapid breathing, muscle pain, loss of consciousness.
The development of hypoglycemia and lactic acidosis increases with the simultaneous intake of alcohol together with Siofor.

Side effects

Possible side effects such as diarrhea, taste of metal in the mouth, nausea, vomiting, anorexia, rarely recorded the development of a blood disease such as megaloblastic anemia, possible allergic reactions in the form of urticaria, rashes.
