Composition

active substances: piperacillin sodium, tazobactam sodium;

1 vial contains sodium piperacillin equivalent to piperacillin 2 g or 4 g and tazobactam sodium equivalent to tazobactam 250 mg (0.25 g) or 500 mg (0.5 g).

Pharmacological properties

This antimicrobial drug is a combination of piperacillin and tazobactam. Combines the properties of an antibiotic and a beta-lactamase inhibitor.
Piperacillin is a semi-synthetic penicillin antibiotic with a broad spectrum of action due to its ability to inhibit the activity of bacteria by inhibiting the formation of the cell membrane and cell wall synthesis.
Tazobactam, in turn, is a derivative of triazolylmethylpenicillanic acid, is a β-lactamase inhibitor that expands the spectrum of piperacillin’s action and enhances its activity against strains resistant to penicillins and cephalosporins.

Indications

This drug Tazpen has a wide range of antimicrobial effects. It is used to treat:

  • upper and lower respiratory tract infections, including pneumonia, including nosocomial and ventilator-associated pneumonia;
  • urinary tract infections, including pyelonephritis;
  • infections of the abdominal cavity, including complicated ones – appendicitis complicated by peritonitis, inflammatory processes in the pelvic organs, including in children aged 2 to 12 years;
  • infections of the skin, soft tissues, bones, joints (including diabetic foot syndrome)
    septicemia.

Tazpen are also used in combination with aminlycosides in the treatment of children and adults suffering from fever caused by neutropenia, which is caused by a bacterial infection.

Contraindications

Not applicable in case of hypersensitivity to penicillins, cephalosporins and beta-lactamase inhibitors.
Also contraindicated for the treatment of children under the age of 2 years.

Application during pregnancy and lactation

This drug is undesirable for use during pregnancy, since there is no complete confidence in its safety for the fetus and mother. As a result, it can be used in a given period of a woman’s life only if the intended benefits of taking it outweigh the possible harm caused to the woman and child.
The drug passes into breast milk, therefore, lactation must be suspended during treatment.

Method of administration and dosage

Tazpen is administered intravenously, as an infusion, for 20-30 minutes or more, or in a jet, slowly, over 3-5 minutes.
The dosage and duration of treatment is calculated by the attending physician and depends on the severity of the infection and the patient’s condition.
The usual dosage in adults (over 12 years old and weighing 50 kg and more) with complicated conditions caused by acute infections is from 2.5 in total to 4.5 in total for 1 administration, the infusion is performed three times a day, with neutropenia – 4 times per day (every 6 hours).
Elderly patients do not need dose adjustment. In renal and hepatic insufficiency, the dose is adjusted depending on the severity of the disease and can be reduced to 8 grams of piperacillin per day.
The dosage for children 2-12 years old with a body weight of less than 50 kg with neutropenia is prescribed at the rate of up to 90 mg per 1 kg of body weight (in total), administered every 6 hours, while combined with amine glycosides (not exceeding 4.5 g in total) every 6 hours.
For complicated infections (acute appendicitis) – the dose in children weighing up to 40 kg and normal kidney function is 112.5 mg per 1 kg of body weight in total piperacillin and tazobactam (but not more than 4.5 grams), administered every 8 hours.

Overdose

Overdose can cause increased side effects, including seizures, nausea, diarrhea.
Treatment of symptoms is recommended (diazepam, barbiturates are recommended for convulsions) and hemodialysis.

Side effects

The most common side effects are:

  • disorders of the gastrointestinal tract (diarrhea, nausea, vomiting, flatulence, anorexia);
  • allergic reactions (rash, eczema, urticaria, anaphylaxis, rhinitis, shortness of breath);
  • neutropenia, thrombocytopenia, leukopenia, eosinophilia, hemolytic anemia;
  • dizziness, headache, increased fatigue, convulsions, chills, tachycardia, arrhythmia, arthralgia, muscle weakness;
  • candidiasis;
  • fever, reactions at the injection site.
