Pharmacotherapeutic group

Highly active diuretics. Simple sulfonamide preparations.
ATX code C0ZS A04.

Pharmacological properties

Torasemide is a loop diuretic. However, at low doses, its pharmacodynamic profile resembles thiazide diuretics in terms of diuresis level and duration. At higher doses, torasemide accelerates diuresis in a dose-dependent manner, and this effect can be very pronounced.

Indications
Trigrim is used to eliminate and prevent the development of edema caused by impaired functioning of the cardiovascular system and renal or hepatic failure. In addition, the drug can be used alone or as part of a complex therapy for primary hypertension.

Contraindications

Trigrim is not used in the treatment of patients with established intolerance to the components of the drug, with exacerbation of renal and hepatic diseases. It is not used in pediatrics, as well as if a person has severe diseases of the cardiovascular system, decompensated diabetes mellitus, gout and cirrhosis of the liver.

Application for pregnancy and lactation

Trigrim is strictly forbidden to use in the therapy of pregnant and lactating women.

Method of administration and dosage

Trigrim is taken orally, regardless of food intake and time of day. The tablets are taken whole with a small amount of water. The starting dose for the treatment of edema is 5 mg r / s. If necessary, the dose of the drug can be increased to 20 mg.
In the treatment of edema that has arisen against the background of congestion in chronic heart failure, the maximum daily dose of Trigrim can be increased to 200 mg. In the treatment of chronic renal insufficiency, the starting dose can range from 20 to 200 mg per day.
In the treatment of liver cirrhosis, the starting dose may range from 5 to 10 mg of the drug per day. If necessary, the amount of the drug can be increased to 40 mg per day.
Treatment of primary arterial hypertension begins with 2.5 mg of the drug with a gradual increase in the dose to 5 mg per day. A further increase in dosage in the treatment of hypertension is meaningless, since it does not enhance the therapeutic effect of the drug, but it increases the possibility of developing allergic reactions.

Overdose

Taking Trigrim in high doses can cause the development of clinically significant symptoms of intoxication, such as forced urination, accompanied by electrolyte imbalance and hypovolemia. Also, the patient may experience increased drowsiness, slight confusion, dyspeptic disorders and collapse.
There is no specific antidote at the moment, and therefore the patient is rinsed with the stomach, provided with an abundant drink and enerosorbents.

Side effects

When treating Trigrim, patients may experience persistent hypotension, thromboembolism, increased activity of liver enzymes, dyspeptic disorders, pancreatitis, acute urinary retention, headaches, weakness, fatigue, convulsions, limb paresthesias, hearing loss, visual impairment, tinnitus and allergic reactions.
