Composition

active substance: tropicamide;

eye drops 1%: 1 ml of the drug contains tropicamide 10 mg in terms of 100% dry matter;

eye drops 0.5%: 1 ml of the drug contains tropicamide 5 mg in terms of 100% dry matter;

Excipients: edetate disodium, benzalkonium chloride, sodium chloride, dilute hydrochloric acid, water for injections.

Pharmacological properties

An anticholinergic drug that blocks the M-cholinoreceptors of the pupillary sphincter and ciliary muscle, causing mydriasis. Tropicamide acts similarly to atropine, but the cycloplegic and mydriatic effects occur more rapidly and are short-lived.

0.5% solution can be used to achieve mydriasis with minor cycloplegia. The use of 1% solution also causes accommodation paralysis.

Preclinical studies of pharmacological safety, repeated dose toxicity, genotoxicity, carcinogenic potential, reproductive toxicity did not reveal any risk to humans. Toxic effects were observed only at doses exceeding the maximum dose for humans, and their occurrence is unlikely in clinical use.

Indications

Tropicamide is used for diagnostic purposes when mydriasis or cycloplegia is necessary, as well as when conducting a study of the fundus and determining refraction. It is also prescribed in cases where the patient has an intolerance to other drugs used to dilate the pupil. In addition, it is used as part of the complex therapy of inflammatory processes and to eliminate eye adhesions – the fusion of eye tissues with the surrounding tissues.

Contraindications

Tropicamide is not used for intraocular hypertension, angle-closure glaucoma, and intolerance to the components of the drug. The drug must be used with caution in the treatment of children under one year old, lactating and pregnant women.
It is not recommended to use contact lenses during the use of Tropicamide, as benzalkonium chloride, which is part of it, is deposited on the surface of soft contact lenses. When using the drug, you can use hard contact lenses, but it is recommended to remove them when instilling the eyes.
To prevent the adsorption of the solution and reduce the likelihood of developing systemic side effects, after instillation for 1 or 2 minutes, you need to lightly press your fingers on the area of ​​the lacrimal sac.

Application during pregnancy and lactation

Tropicamide can be used in the treatment of pregnant and lactating women if the potential benefit of its use outweighs the risk to the newborn and fetus.

Method of administration and dosage

Tropicamide is used for topical use only. Eye drops of 0.5% are prescribed to patients in 1 or 3 drops in the conjunctival sac of the eye. To achieve the maximum therapeutic effect, the frequency of administration of the drug is carried out up to 6 times a day. When determining refraction, 6 instillations are carried out every 6 or 12 minutes; for diagnostic pupil dilation, instillation is carried out 1 to 3 times over 10 minutes. 1% eye drops are used in the study of refraction to obtain the effect of cycloplegia, for this, the drug is first instilled in 1 or 2 drops into the conjunctival sac, and after five minutes the manipulation is repeated. In that case, if they ate 25 minutes after instillation, the study was not carried out, then to prolong the mydriasis, the patient is instilled another 1 drop.

Overdose

With the local use of Tropicamide, an overdose is unlikely, but when the drug is taken orally, patients experience dry skin and mucous membranes, convulsions, agitation, respiratory paralysis and coma.
To eliminate the symptoms of poisoning, the patient is given gastric lavage, activated carbon is prescribed, and if seizures occur, intravenous diazepam is administered.

Side effects

With prolonged use of Tropicamide in overestimated doses in patients, there is an increase in intraocular pressure, violation of accommodation, photophobia. It is also possible the occurrence of such systemic effects as lowering blood pressure, tachycardia, headaches, hyperthermia, dry mouth, allergic reactions.
