Agrel (clopidogrel) is used for prevention of atherothrombosis, prevention of atherothrombotic and thromboembolic events in atrial fibrillation.

Clopidogrel is prescribed at a dose of 75 mg 1 time per day, regardless of food intake.

Composition

active ingredient: clopidogrel;

1 tablet contains clopidogrel besilate 111.85 mg, equivalent to 75 mg clopidogrel

Excipients: macrogol 6000, microcrystalline cellulose (E 460), crospovidone, hydrogenated castor oil film shell (ethylcellulose (E 462), macrogol 6000, titanium dioxide (E 171)).

Indications.

Prevention of atherothrombosis in adults:

  • patients with myocardial infarction (onset of treatment within a few days but not later than 35 days after onset), ischemic stroke (onset of treatment after 7 days, but not later than 6 months after onset), or patients with who have been diagnosed with peripheral artery disease;
  • patients with acute coronary syndrome:
    acute coronary syndrome without ST-segment elevation (unstable angina or myocardial infarction Q), including patients who have been stented during percutaneous coronary angioplasty in combination with acetylsalicylic acid (aSA);
    with acute myocardial infarction with ST-segment elevation, in combination with acetylsalicylic acid (in patients receiving drug treatment and shown thrombolytic therapy).

Prevention of atherothrombotic and thromboembolic events in atrial fibrillation

Clopidogrel in combination with ASA is indicated in adult patients with atrial fibrillation who have at least one risk factor for vascular events, in whom there are contraindications to treatment with vitamin K antagonists (VKAs) and who have a low risk of bleeding, for the prevention of atherothrombotic and thromboembolic events , including stroke.

Contraindications.

  • Hypersensitivity to the active substance or to any component of the drug.
  • Severe liver dysfunction.
  • Acute bleeding (eg ulcer or intracranial hemorrhage).

Method of application and dose.

Adults and elderly patients

Clopidogrel is prescribed at a dose of 75 mg 1 time per day, regardless of food intake.

In patients with acute coronary syndrome:

Non-ST elevation acute coronary syndrome (unstable angina or non-Q wave myocardial infarction): treatment with clopidogrel begins with a single loading dose of 300 mg, and then 75 mg of clopidogrel 1 time per day (with acetylsalicylic acid (ASA) at a dose of 75-325 mg per day). Since the use of higher doses of ASA has been associated with a high risk of bleeding, it is recommended not to exceed the dose of ASA 100 mg. The optimal duration of treatment has not been officially established. The results of studies indicate in favor of the use of up to 12 months, and the maximum effect was observed after 3 months of treatment;

– acute myocardial infarction with ST-segment elevation: clopidogrel is prescribed 75 mg 1 time per day, starting with a single daily loading dose of 300 mg in combination with ASA, with or without thrombolytics. Treatment of patients older than 75 years begins without a loading dose of clopidogrel. Combination therapy should be started as soon as possible after the registration of symptoms and continued for at least 4 weeks. The benefit of combining clopidogrel with ASA for more than 4 weeks in this disease has not been studied.
