PHARMACOLOGICAL GROUP
Antimicrobial agents for systemic use. Other antimicrobial agents. ATX code J01X X01.
PHARMACOLOGICAL PROPERTIES
Pharmacological. Phosphocin has a broad spectrum of antibacterial action. It is effective against strains of causative agents of urinary tract infections, most often isolated:
E. coli, Citrobacterspp., Klebsiellaspp., Proteusspp., Serratiaspp., P. aeruginosate Enterococcus faecalis – even if they are resistant to other antibacterial drugs. The drug also acts on penicillinase-forming strains. Phosphocin has an antiadhesive effect on bacteria that adhere to the epithelium that lines the urinary tract.
Pharmacokinetics. Phosphocin is easily absorbed from the gastrointestinal tract, the maximum concentration in blood plasma after 2:00 is 30 – 35 μg / ml, the half-life is 2:00. Phosphocin is not metabolized in the body; it is excreted by glomerular filtration in a biologically active form. Phosphocin accumulates mainly in the kidneys at a concentration of 2500-3500 μg / ml, and its minimum inhibitory concentration for E. coli is 128 μg / ml. Enterohepatic circulation of Phosphocin allows maintaining the therapeutic level of the drug for 48 hours – this time is enough for urine sterilization and recovery.
INDICATIONS
Treatment of acute uncomplicated urinary tract infections caused by microorganisms sensitive to fosfomycin in adult men, women and girls aged 12 years and older. Prophylaxis during diagnostic procedures and surgical interventions in adult patients.
CONTRAINDICATIONS
INTERACTION WITH OTHER DRUGS AND OTHER INTERACTIONS
Simultaneous reception with metoclopramidom with other drugs that increase the motility of the gastrointestinal tract, reduces the absorption of the drug, which leads to a decrease in the concentration of Phosphocin in serum and urine.
FEATURES OF APPLICATION
The use of broad-spectrum antibiotics, including fosfomycin trometamol, can lead to antibiotic-associated colitis (including pseudomembranous colitis). Therefore, it is necessary to take into account the likelihood of this diagnosis in patients with severe diarrhea during or after taking fosfomycin trometamol. If the diagnosis is confirmed, appropriate treatment should be started immediately. In this case, drugs that inhibit peristalsis are contraindicated.
