Symbicort Turbuhaler, 160 mcg/4.5 mcg, is prescribed to adults and children over 12 years of age for the regular treatment of bronchial asthma in case of feasibility of using combined therapy (inhaled corticosteroid and long-acting β2-adrenoceptor agonist).

Composition
active substances: 1 inhalation (1 dose) contains: 160 mcg micronized budesonide;
4.5 mcg of formoterol fumarate dihydrate;
excipient: lactose, monohydrate.

Powder for inhalation, dosed.
The main physical and chemical properties:
inhaler: red rotating dispenser. A Braille code is embossed on the rotating dispenser. The cap is white. There are five ribs inside the lid.
The number 60 is visible in the dosage indicator window. The nozzle has four rods and can rotate.
Content: content from white to almost white, mostly in the form of rounded granules.

Pharmacological properties
This drug is used in the treatment of obstructive diseases of the lower respiratory system.
Formoterol fumarate, one of the active active ingredients, is a selective agonist of beta2 adrenergic receptors, which is characterized by a long-term effect that occurs soon after its use (within 1-3 minutes).
If the drug is used in the form of inhalation, then within a few minutes the smooth muscles of the bronchial tree relax, which lasts for half a day.
This provides sufficient control over breathing in the lower airways, which is especially important for those patients who suffer from reverse obstruction of the bronchi and lungs.
Budesonide is a glucocorticosteroid that, when inhaled, enters the trachea, bronchi and lungs, where it produces an anti-inflammatory effect.
As a result, the symptoms of bronchial asthma are weakened.
With local exposure, budesonide is more effective than with systemic use.
Indications
This drug Simbicort Turbuchaler is prescribed for the treatment of bronchial asthma, COPD, if the patient is indicated for treatment with corticosteroids and beta-2-agonists.
Contraindications
This drug is not used if the patient is allergic to one of its constituent substances, including auxiliary substances.
For the treatment of children can be used from 6 years of age.
Application during pregnancy and lactation
This drug can be used to treat pregnant women only if their life is at risk and with careful weighing of the benefit / harm ratio by the attending physician.
Lactation is suspended for the duration of treatment with this drug.
Method of administration and dosage
The introduction of the drug Simbicort Turbuchaler occurs by inhalation of a dose of the drug in the form of microscopic doses of powder, using an inhaler. The use of the inhaler is explained in detail in the attached instruction sheet.
The recommended dosage is 1-2 doses 2 times a day.
It is advisable to switch over time to the minimum effective dose, inhaling 1 dose once a day.
In order for the drug to enter the windpipe when inhaling, you need:
- shake the inhalation can;
- exhale the air from the lungs as much as possible;
- by pressing the buttons of the inhaler, take a deep breath;
- hold your breath for as long as possible.
If the patient previously had nocturnal attacks of bronchial asthma, then this 1 dose is inhaled before going to bed. If daytime attacks are more frequent, then the dose of the drug is inhaled in the morning.
After use, rinse your mouth with water to avoid oral candidiasis.
Elderly patients and patients with impaired renal or liver function do not need a special dosage adjustment.
Overdose
Overdose with this drug is unlikely. Prolonged excessive intake of the drug inside can theoretically cause hypokalemia, tremor, adrenal dysfunction, which is reversible.
In this case, gastric lavage and sorbent intake are recommended.
The antidote is unknown.
Side effects
The drug can cause side reactions in the form of:
- oral candidiasis;
- allergic reactions – angioedema, rash, shortness of breath, anaphylactic shock;
- oppression of the adrenal glands, reduction of bone mineralization, growth retardation in children;
- hypokalemia, hyperkalemia, hyperglycemia;
- nausea, dyspeptic symptoms;
- nervousness, insomnia;
- headache, tremors of the limbs;
- cataracts, glaucoma;
- tachycardia, arrhythmias;
- bruising;
- muscle spasms, arthralgia, myalgia, limb fractures.
