Description
Medetrom composition
active substances: tobramycin, dexamethasone;
1 ml of suspension contains tobramycin 3 mg and dexamethasone 1 mg;
Excipients: tyloxapol, hypromellose, anhydrous sodium sulfate, sodium chloride, disodium edetate, benzalkonium chloride, sulfuric acid or sodium hydroxide, purified water.
Dosage form
Eye drops, suspension.
Pharmacological properties
Combined local preparation Medetrom, used in ophthalmology, which contains dexamethasone and tobramycin as active ingredients.
Dexamethasone is a monofluorinated synthetic glucocorticoid (adrenal cortex hormone), has anti-inflammatory, anti-allergic, antipruritic properties. Does not have a mineralocorticoid effect.
Tobramycin is a broad-spectrum antibiotic that has bactericidal properties, affecting a significant number of pathogenic organisms and inhibiting bacterial protein synthesis.
Indications
It is used in ophthalmology for the treatment of non-infectious inflammatory and allergic diseases of the conjunctiva, cornea and anterior segment of the eye, sensitive to steroids.
Also used if there is a superficial bacterial infection or the risk of its development.
Including the reactions of inflammation of the eyes that occur in the postoperative period with eye injury.
Dexamethasone, an inhibitor of one of the phospholipase enzymes, is capable of inhibiting the synthesis of arachidonic acid. It also has the ability to prevent the migration of leukocytes, phagocytes, the formation of kinins, the production of antitoxins, and reduce the permeability of blood vessels.
Hypromellose, which is part of the drug as an auxiliary drug, has the ability to increase the viscosity of the drug and thus delay the outflow of drops, prolonging their contact with the eye surface, improving the bioavailability of the drug and providing an optimal environment for its action.
Contraindications
It is not used if the patient is at increased risk of allergy to one of the components of this medicinal product
Contraindicated if the patient has:
- acute untreated bacterial, fungal, viral infections, chickenpox or vaccinia;
- corneal injury or ulcer;
- mycobacterial eye infections;
- glaucoma.
Wearing contact lenses is not recommended during treatment with this drug.
Application simultaneously with non-steroidal anti-inflammatory drugs can lead to inhibition of corneal wound healing.
Children
It is not used in pediatrics under the age of 1 year.
Application during pregnancy and lactation
The use of this drug is contraindicated both during pregnancy and during lactation.
Method of administration and dosage
Usually, 1-2 drops of Medetrom are instilled every four to six hours. Instillation is performed in the conjunctival sac of each eye.
It is used only for instilling the eyes. Before use, shake the bottle vigorously for one to two minutes.
When instilling, it is necessary to ensure that the tip of the bottle does not touch the surface of the eye. The procedure must be carried out with cleanly washed hands.
The duration of treatment is determined by the attending physician, based on the course of the treatment process, the individual characteristics of the patient’s physiology.
If the desired effect is not achieved after 3-4 days, the appointment of additional systemic or conjunctival therapy is acceptable.
The duration of treatment usually does not exceed 2 weeks. If after the first two days of therapy there is no noticeable improvement, the treatment should be changed.
Overdose
Overdosage has not been observed or described to date.
Theoretically, overdose can cause increased side effects (punctate keratitis, erythema, increased lacrimation, edema and itching of the eyelids).
If you get an excess of the drug in your eyes, you should wash your eyes with clean water.
In case of accidental ingestion, it is necessary to rinse the stomach, take sorbents.
Side effects
Using this drug may cause a hypersensitivity reaction, the symptoms of which are:
- itching;
- edema;
- erythema of the conjunctiva.
- Also, the use of this medication can lead to an increase in pressure inside the eye, the development of glaucoma. In addition, it may develop:
- eye discomfort;
- dysgeusia;
- photophobia;
- irritation, redness of the eyes;
- increased intraocular pressure;
- decreased visual acuity;
- ptosis of the eyelids;
- punctate keratitis;
- mydriasis, pain in the eyes.
Long-term topical use of this drug may in some cases lead to the development of thinning of the cornea, the risk of perforation.
Storage conditions and periods
Stored Medetrom for up to 3 years at a temperature not exceeding 25 ° C.
After opening the bottle, the drug retains its properties for one month.
