Pharmacological properties
Acnetin (isotretinoin) capsules Pharmacodynamics.
Isotretinoin is a stereoisomer of transretinoic acid (tretinoin). The exact mechanism of action of isotretinoin has not yet been determined in detail, however, it has been established that the improvement in the clinical picture of severe forms of acne is associated with a decrease in the activity of the sebaceous glands and a histologically confirmed decrease in their size. In addition, the anti-inflammatory effect of isotretinoin on the skin has been proven.
Efficiency
Hyperkeratosis of the cells of the epithelium of the hair follicle and the sebaceous gland leads to peeling of corneocytes in the duct of the gland and blockage of the latter with keratin and excess sebaceous secretion. After that, a comedone is formed and, in some cases, an inflammatory process joins. Aknetin® inhibits the proliferation of sebocytes and acts on acne by restoring the normal process of cell differentiation. Sebum is the main substrate for the growth of Propinibacterium acnes, reducing sebum production inhibits bacterial colonization of the duct.
Indications
Severe forms of acne (including nodular and conglobate acne, acne with a tendency to permanent scarring), not amenable to standard methods of treatment (systemic antibiotic therapy, topical treatment).
Contraindications
- Pregnancy and lactation period;
- in women of reproductive age if all the conditions of the “Pregnancy Prevention Program” are not met;
- hypersensitivity to isotretinoin or to any components of the drug;
- liver failure;
- severe hyperlipidemia;
- hypervitaminosis A;
- concomitant therapy with tetracyclines.
Due to the fact that Acnetin (isotretinoin) capsules contain soybean oil, the drug is contraindicated for patients with peanut and soy allergies.
Interaction with other medicinal products and other forms of interaction
Simultaneous administration of Aknetin ® and vitamin A should be avoided due to the possible worsening of symptoms of hypervitaminosis A. Cases of a benign increase in intracranial pressure (pseudotumor of the brain) have been reported with the simultaneous use of isotretinoin with tetracyclines. Therefore, simultaneous use with tetracyclines should be avoided. Combined use with local keratolytic or exfoliative drugs for the treatment of acne is contraindicated due to the possible increase in local irritation.
