Oxitocin is used to stimulate and enhance the contractile activity of the uterus. It can be used in the prenatal period, for the induction of labor, in particular, for: arterial hypertension (preeclampsia, eclampsia); erythroblastosis of the fetus, maternal or gestational diabetes mellitus; prenatal bleeding or the need for early delivery; premature rupture of the membranes and the impossibility of spontaneous labor. Also used for the planned initiation of labor with oxytocin, with: post-term pregnancy (more than 42 weeks of gestation); intrauterine fetal death or with intrauterine fetal growth retardation.
Description
Oxitocin composition
Main active ingredients: oxytocin.
Excipients: chlorobutanol hemihydrate, water for injection.
Release form
It is produced in the form of a solution for injection, in ampoules of 1 ml. 1 ml contains 5 IU of oxytocin.
Pharmacological properties
This drug Oxitocin belongs to the group of drugs – pituitary hormones, used systemically.
It is also effective in the developed syndrome characterized by the appearance of attention deficit disorder, including if the patient has hyperactivity.
The main active ingredient of the drug is oxytocin. This is a drug whose properties are similar to the endogenous oxytocins of the pituitary gland (secreted by its posterior lobes).
The uterine myometrium has similar substances – oxytocin receptors. Oxytocin has the property of contracting uterine smooth muscles due to the property of increasing intracellular calcium concentration, which stimulates the appearance of contractions similar to spontaneous physiological labor, and increases the frequency and strength of contractions.
In addition, the drug causes a temporary decrease in uterine blood flow by increasing it in the kidneys and blood vessels in the brain.
Oxytocin also stimulates the secretion of breast milk.
Indications
Oxitocin is used to stimulate and enhance the contractile activity of the uterus.
It can be used in the prenatal period, for the induction of labor, in particular, for:
- arterial hypertension (preeclampsia, eclampsia);
- erythroblastosis of the fetus, maternal or gestational diabetes mellitus;
- prenatal bleeding or the need for early delivery;
- premature rupture of the membranes and the impossibility of spontaneous labor.
Also used for the planned initiation of labor with oxytocin, with:
- post-term pregnancy (more than 42 weeks of gestation);
- intrauterine fetal death or with intrauterine fetal growth retardation.
It can also be used to enhance the contractile activity of the uterus during prolonged childbirth, the threat of abortion or incomplete abortion, as well as in the diagnostic assessment of the embryonic-placental respiratory reserve of the fetus during pregnancy with high risk.
Contraindications
The drug in question is contraindicated for use in cases where the patient has severe hypersensitivity (allergy) to the main or to one of the auxiliary components.
Use with caution when:
- narrow pelvis, abnormal position of the fetus;
- urgent obstetric emergencies when an urgent caesarean section is required;
- severe diseases of the cardiovascular system;
- other contraindications.
Application during pregnancy and lactation
This drug Oxitocin is used for certain indications for delivery.
Method of administration and dosage
The dosage, regimens and duration of treatment are determined by the attending physician individually, depending on the nature and course of the disease, with constant monitoring of the condition of the fetus and mother.
Saline is pre-introduced. The initial dose does not exceed 0.5-4 medical units of oxytocin per minute. The dose may be increased every 20-40 minutes by 1-2 IU.
In preterm labor, the rate of administration may exceed 20 IU per minute.
Overdose
Overdose symptoms are expressed according to the degree of sensitivity of the uterus of a particular patient.
Overdose can cause:
- strong or prolonged contractions;
- promiscuous labor;
- rupture of the body, cervix, vagina;
- fetal hypoxia;
- fetal death.
Treatment – urgent cessation of drug administration, resuscitation measures.
Side effects
Adverse reactions may occur. In this case, such reactions are possible as:
- thrombocytopenia;
- anaphylactic reactions, overhydration;
- arrhythmia, tachycardia;
- nausea, vomiting;
- death of childbirth, postpartum hemorrhage, uterine hypertonicity;
- uterine spasms;
- hemorrhage in the retina of the eyes in newborns;
- arrhythmia, tachycardia;
- asphyxia, fetal death caused by asphyxia.
